I agree to pay the required fees. Full payment (no part payments allowed) must be paid within the first 14 days of every applicable 9 week term.

Fees paid after the 15th day after the first class of the term incur a $10 per month late fee.

A casual class must be paid in full on day of or prior to class.

Wednesday classes must be booked using the online booking system as numbers are limited.

Please sign in at reception if you are not a gym member.

Gym members receive discounted classes but will need show proof of valid membership.

Any debt collection fees will be the responsibility of the enrollee. There will be no refunds for absences or public holidays. Term fees may be paid by cash in a named envelope or direct credited into ANZ account: 01 0819  0363512 01 with the dancer's name as a reference.

Her Courage is not responsible for any damages caused by the enrollee or any medical treatment required while undertaking this activity. This will be the responsibility of the enrollee or their legal guardian.

MEDIA CONSENT

I give permission to photograph and record me for use in audio, video, film, or any other electronic, digital, or social media outlet.
